Rep Tryouts - Permission to Tryout (PTS) Forms

All players trying out for Competitive (Rep) teams away from their home association require a Permission to Tryout (formerly Permission to Skate) form, before they can participate.  Please note the below information for FGHA players hoping to try out for other associations.  

FGHA registered players planning to try out for other Associations: 

• PTS forms for FGHA players can only be signed by either Kevin Smoke (FGHA Rep Director) or Jason Bonneville (FGHA VP Rep).   
• Please allow 24 hours minimum for the FGHA to action the form and return it to you. 
• Download, fill out and print off 2 copies of the 2022-23 OWHA PTS (Permission to Tryout) form.
• Please email Jason and Kevin with the completed form and ask them to sign off. 
• NOTE: All team and association fees must be paid in full before the PTS form can be signed.  If all fees are not paid in full, the PTS form will not be approved. 

Players coming from other associations:

• Download, fill out and have the PTS (Permission to Tryout) form signed off by your home association prior to arrival at the rink. 
• You will need to bring your signed PTS form to the rink for the first skate.
