• Players who are intending to try out for Rep teams with a different association other than the one they were last registered for must follow the Permission to Tryout (PTT) process.  This form was previously known as the PTS (Permission to Skate). 

  Permission to Tryout forms are valid as of 12:01 am on April 14th, 2026 to May 31st, 2026 and from August 14th, 2026 until a date to be announced by the OWHA.

  Players last registered with another association, and wishing to try out for an FGHA Rep team:

  • Complete the OWHA Permission to Tryout form
  • Follow your current Association’s process to have the Designated Executive Member sign and date the form.
  • Print the completed and signed form and bring to your first tryout with the FGHA. 

  Players last registered with the FGHA wishing to try out with another association:

  • Complete the OWHA Permission to Tryout form
  • Email the completed form to FGHA Rep Director Paula Caverson
  • Please allow 24-48 hours turn-around to receive your signed PTT form
  • All fees (both team and association) must be paid in full before the FGHA will release a player to another association
  • Notes: Form requests are to be made via email only and the FGHA Rep Director is the only member of the FGHA Board of Directors authorized to sign the PTT form.

For FGHA Players Needing a PTT Form:

• The Rep Director is the only authorized individual to sign on behalf of the FGHA.
• You will need to provide:
  • Two completed forms for each association you wish to try out for.
  • Association and division/level you are trying out for.
  • Dates of the tryout(s).
  • Email the completed form to FGHA Rep Director Paula Caverson
  • Allow 24-48 hours for processing.
  • All fees (both team and association) must be paid in full before release to another association.
